Adult Supported Living Services

 

Supported living services are individually tailored arrangements of resources and supports that enable people to live as independently as possible in their community.

 

Who is Eligible?

This service is intended for adults (age 18 and over) who have a developmental disability.  Some individuals are able to live with support in their own apartment; others may remain living with their families or have a roommate.

 

How to access services?

Supported living services are accessed through a developmental disabilities Case Manager or DEAP Family Support Specialist who can place an individual on the waiting list.  When a service opportunity becomes available, the individual with the highest need for the service will be selected by a screening committee.  Individuals ages 18-21 years old receiving Intensive Family Education & Support Services may use their cost plans to purchase these or other adult services without being on a waiting list.  DEAP provides services in the following counties: Carter, Custer, Dawson, Fallon, Garfield, Powder River, Prairie, Rosebud, Treasure and Wibaux.

 

Services Provided:

A team, with the individual, develops an Individual Plan outlining the individual’s long-term goals and annual objectives.  The type of service provided is based on the individual’s strengths, needs, and preferences.  Services provided may include:

  • Personal care services such as assistance with personal hygiene, dressing, eating, and ambulatory needs

  • Residential habilitation- acquiring skills in activities of daily living such as personal grooming, household chores, and food preparation

  • Day habilitation-training in the use of community services, basic life skills, appropriate skills for the workplace and social skills

  • Supported employment- assistance getting and keeping paid employment

  • Respite- services provided to the individual to relieve those persons normally providing the care

  • Environmental accessibility adaptations such as ramps to accommodate wheelchairs

  • Adaptive equipment- devices which increase the individual’s independence

  • Private Duty Nursing

  • Respiratory Services

  • Meals Services

  • Dietician Services

  • Physical, Occupational, or Speech Therapy

  • Psychological Services

  • Transportation for the individual to gain access to services, activities and resources

  • Homemaker services- meal preparation and routine household care when the person responsible for these activities is temporarily absent or unable to manage the home and care for himself/herself or others.
