DEAP's Disability Services Include:

 

   

 IDEA Infant & Toddler (Part C)

Part C is a program of early intervention services for children with disabilities ages 0-36 months. Families with children who meet the eligibility criteria established by the state are entitled to a range of services defined under federal law. Services are delivered as part of an Individual Family Service Plan (IFSP) developed in cooperation with the family. Services available include: Family Support Coordination; Assistive Technology; Audiology; Family Education, Counseling and Home Visits; Nursing; Health Related; Medical; Nutrition; Occupational, Physical and Speech Therapy; Psychological; Vision; Social Work; Special Instruction; and Transportation.

 Who is eligible?

 Children from birth to age three with the following criteria:

  • Diagnosed physical or mental condition that has a high probability of resulting in developmental delay
  • 50% developmental delay in one developmental area or a 25% delay in two or more areas of development, including:physical (motor, vision and hearing), cognitive (thinking ability), communication (Speech and language), social-emotional (interaction with others) or self-help skills.

How is a referral made?

Any concerned person, parent, or agency may make a referral about a child who may appear to have a delay or need services to the Intake Coordinator. Parental or legal guardian permission is required to make a referral.

 Family Education & Support

Family Education & Support provides home-based services to families who have children with developmental disabilities or who are at risk for delays. A Family Support Specialist assists the family members with information, training, emotional support, and coordination of other services.    

Who is eligible?

  • Children who are developmentally delayed or at risk because of prematurity, chronic medical problems, behavior problems, lack of development, or other concerns may be served up to their sixth birthday
  • Children who have a developmental disability may be served up to their 22nd birthday
  • Selection for service is based on priority of need after being placed on a waiting list

How is a referral made?

By calling the Intake Coordinator with parent or legal guardian approval

 

  Children's Waiver Services

Children's Waiver Services (CWS) are designed to provide resources and supports to meet the needs of famiies whose children have extraordinary medical or behavioral heeds as well as a developmental disability. CWS is a Medicaid waiver program, which means the child served can receive Medicaid without considering family income. When a CWS opportunity becomes available in the region, a child with the highest neeeds is selected from a waiting list.

 

Who is eligible?

Individual if at risk of out-of-home placement without additional resources, is under 22 years of age, and has one or more of the following:

  • Severe or profound mental retardation including extreme dficiencies in self-care and daily living skills as compared to peers
  • Significant maladaptaive social and/or personal behaviors which require intervention
  • Severe medical or health related problems

By calling the Intake Coordinator with parent or legal guardian approval

  Community Supports & Supported Living 

Community Supports and Supported Living provide individualized resources and supports to meet the health and safety needs of people, and to help them reach their goals. Individuals may live with their families or independently with some support.

Who is eligible?

Adults age 18 and over who have a developmental disability.  

How is a referral made?

A Case Manager or DEAP Family Support Specialist can place an individual on the waiting list. Selection for service is based on priority of need after being placed on a waiting list.

  PASARR - Preadmission Screening and Annual Resident Reviews 

PASARR provides services to nursing facility residents or applicants. Evaluations are provided to determine if individuals have mental retardation or related conditions and a recommendation is made for the most appropriate placement. Annual Resident Reviews are provided to assess the ongoing needs and provide specialized services.

 Sky Reach Youth Home

Sky Reach is located in Glendive, MT. It is a licensed residential facility for individuals who have severe disabilities. A homelike atmosphere is maintained while individuals receive physical care and training in the areas of self-help skills and social and community integration.
