have mental retardation or related conditions and to applicants of nursing

What
services are provided?
Services include screens to determine level of care and if an applicant has
mental retardation or related conditions and needs specialized services.
Annual resident reviews are completed for each nursing facility resident to
review and update the needs for specialized services if the placement is
appropriate and to develop a specialized service plan if needed.
Other services include "subsequent screens" when a condition may have changed
for a previous screen and "modified screens" completed by a Quality Improvement
Specialist (QIS) when an individual is already in community services provided by
the Developmental Disabilities Program. |
